Each record carries its date, provider, condition, treatment and the page it came from, including the full span when a record runs across several pages. Name a document once (“Service Treatment Records 1989–1996”) and that name appears in every citation after. A handwritten note that runs onto a later page keeps its date and the doctor’s impression.
The same pages often turn up in the C-file and in separately supplied service records. Copies are grouped for review, and merging them keeps one record that lists where every copy was found.
Rating decisions in the file are found and read, including VA’s stated reasons for each issue. Copies are merged. Decisions the file mentions but doesn’t contain are listed, so you know what to request.
Social Security numbers are removed before anything is saved. Uploaded files are read and not kept; the records extracted from them are stored with the case.
